CRITERIA FOR HUMANA CONTRIBUTION SELECTED BY AWARDEE
Since 2012, the C. Colburn Hardy
Older Advocate Award has been accompanied by a
contribution from Humana to a relevant Florida-based
nonprofit organization selected by the awardee. Humana,
Florida's largest Medicare health benefits company with
more than 480,000 members statewide, has long been a
supporter of programs that recognize the volunteer work
of seniors in the community. Below are guidelines the
awardee must consider when selecting the organization
which will receive the Humana funds.

Organization
must be located in the State of Florida

Organization
must be a 501(c)(3) registered nonprofit organization
-
Organization
must be one that develops or manages programs or
services leading to advocacy for a greater awareness or
improvement of the health, needs and well being of
seniors in the community

Organization
must be in good standing within the Awardee's community
or State
-
Contribution may
be divided among up to two Organizations
