CCHOAA CRITERIA

The C. Colburn Hardy Older Advocate Award honors an individual who practices the values articulated by C. Colburn Hardy when he wrote:  “Hopefully, some of the projects in which I was involved helped to make the world a better place in which to work, to live, to love and to have our being.” 

The individual nominated should meet most, if not all, of the criteria for the Award:

An individual volunteer, preferably an older citizen, residing fulltime in the State of Florida.*

An individual who has consistently made major and significant contributions to policies, programs and services  for older Floridians.  Activities could range from policy-making to new program development to significant service improvements and have statewide impact.

An individual who advocates for services and programs for seniors in Florida, in both the public and private sector.

 An individual who demonstrates vision, advocacy and leadership regarding the future of services for the growing aging population in the State of Florida.

* NOTE:  For all nominees, their advocacy efforts on behalf of seniors in Florida must go above and beyond any job responsibilities.

CRITERIA FOR HUMANA CONTRIBUTION SELECTED BY AWARDEE

Since 2012, the C. Colburn Hardy Older Advocate Award has been accompanied by a contribution from Humana to a relevant Florida-based nonprofit organization selected by the awardee. Humana, Florida's largest Medicare health benefits company with more than 480,000 members statewide, has long been a supporter of programs that recognize the volunteer work of seniors in the community. Below are guidelines the awardee must consider when selecting the organization which will receive the Humana funds.

  • Organization must be located in the State of Florida

  • Organization must be a 501(c)(3) registered nonprofit organization

  • Organization must be one that develops or manages programs or services leading to advocacy for a greater awareness or improvement of the health, needs and well being of seniors in the community

  • Organization must be in good standing within the Awardee's community or State

  • Contribution may be divided among up to two Organizations
